Complete CCSD Parent Forms (Required)

A parent or guardian in Household 1 must complete the Parent Forms annually through the “PowerSchool/Parent Forms” tile in the my.cherrycreekschools.org Parent Portal. Parent Forms are essential for updating and verifying medical needs, accepting district policies, ensuring current contact information, and other important information to support your student(s).

Please complete Parent Forms before the first day of school!

25-26 Parking Pass - $50

Please complete this form to apply for a student parking pass at Cherokee Trail High School. Students must bring a copy of their student driver's license to Check-in or the Lower Security Office (after check-in) before a pass will be issued. Students must display the pass in their vehicle or risk receiving a ticket and fine. The fee is $50 for the year. There will be a lottery for seniors interested in parking in the upper parking lot.

25-26 CT Locker Request

Dear CTHS Families,

Lockers are optional this year and are being offered by request only. Your student will have the opportunity to make a locker request during Check-In on Wednesday, July 30th and Thursday July 31st at the security table.

Students will receive their locker number and combination during the first week of school. Students may also request their locker at the upper security window during the first week of school.

Calculator Rental - $25

Graphing calculators are required for students taking Algebra 2,
as well as all Honors/AP/IB/CE courses. They are HIGHLY RECOMMENDED for all other math courses.

TI-84 Plus graphing calculators can be rented for a cost of $25 for the 2025-26 school year.

There is a $100 fee for any lost calculators
There is a $10 fee for lost calculator covers
There is a $10 fee for lost battery covers

Payments can be made in the following ways:

During Check-In on July 30th and 31st
Visit Bookkeeping during the school-year

2026 Graduation Information

CCSD Cherokee Trail High School Waiver

Families must complete this form to allow students to participate in Class Competitions, Pep Assemblies, Dodgeball, Half-time Activities, Homecoming Tailgate, Winter Dance, After Prom, and Senior Celebration. Please contact Activities with any questions concerning specific details at (720) 886-1940. This form must be completed by a parent/guardian listed in PowerSchool.

CTHS HIGH SCHOOL WAIVER Form

Vision and Hearing OPT OUT form (9th Grade Only)

Vision and hearing screenings are offered and conducted at the beginning of the school year for all incoming 9th graders and students that are new to the district. You may choose, however, to opt your student out of these services for personal/religious reasons or if your student has had a vision/hearing screen recently performed by his or her healthcare provider.

CHEROKEE TRAIL DANCE GUEST POLICY

In the interest of student safety, students who wish to bring a guest from a school other than Cherokee Trail must submit a guest registration form with a copy of their guest’s id by the established deadline. Guest forms are available in the Cherokee Trail Activities Office and on the school website at least one month prior to the dance. All guests must be under the age of 21 and may not be middle school students.

DANCE ELIGIBILITY

It is our desire for our students to be involved at Cherokee Trail High School students. To fully access all of the opportunities available, students must prioritize the academic experience. In order to attend dances and other extracurricular events, students may not have more than one failing grade, must be attending classes regularly and should meet behavior expectations outlined in the student handbook. In addition, all fines and fees must be paid in order to purchase dance tickets.

Homecoming Dance Guest Registration

If you would like to bring a guest who does not attend Cherokee Trail High School to the Homecoming Dance, please complete this form and submit to Activities, or email the completed form to CTActivities1@gmail.com. Form must be submitted by Wednesday, August 27, 2025 by 3:45pm.
